Welcome to the Lily’s Pad family!  We are excited to have you come and join us for play!  This form was designed to help us serve your family better and to gather the information we need to get you set up for membership.  Membership is free for any qualifying child as well any siblings, parents or caregivers.  If you have any questions, please contact Dawn at dawn@lilyspadaz.org or call us at 480-576-7740

Many of our grants require us to track which zip codes we are serving in Arizona. Names will not be shared when tracking zip codes.

Please note: This waiver is not required for play at Lily's Pad.
