Forms for Play
Whether you are a member of Lily’s Pad or visiting with one of our nonprofit partners, you will need to have a liablity waiver on file before you can play. (We need one for any child playing and any adult entering our cafe or playground space during play hours.) You can download the forms here and submit signed forms to [email protected]. New members will be sent a copy of these forms via email to be electronically signed. Paper copies are also available on-site and can be completed in person if preferred.
Photo Waivers
We take photos and/or videos during some of our play sessions or special events to provide memories for our families. These images may also be used on our social media, website or advertising materials to share with potential or current donors and community partners. Although not required for play, we appreciate your permission to share the joy and fun that the Lily’s Pad facility provides with our sponsors.
